Before teaching you I need to have some general and medical information about you. If you are able
to print out a Health Questionnaire and complete it prior to attending your first yoga class that would be very
helpful.
There are two versions of the form - one for General Yoga Classes, and one for Pregnancy
Yoga Classes - please make sure that you select the correct one!
